Health Records

Overview​

The Health Records section gives you access to your complete medical history from all connected hospitals — labs, medications, conditions, allergies, procedures, visits, immunizations, and medical documents — all in one place.

Every record shows a colored left border and a source badge so you always know which hospital it came from.


Lab Results​

Summary Cards​

What You See​

Each row shows the test name, your result value, the normal range, the result status, which hospital it came from, and the date.

Result Status​

BorderlineYellowYour result is close to the edge of the expected range

How to Filter​

  • Status tabs — view All, Normal, High, Low, or Borderline results (each tab shows a count)
  • Date range — Last 30 days, Last 3 months, or Last year
  • Category — filter by test category (e.g. Chemistry, Hematology)
  • Search — type part of a test name

Trend Chart​

Click any row to see a chart of how that test value has changed over time. The chart shows the normal range as a shaded zone so you can easily see when values were in or out of range.

Lab Result Detail​

Click a row to also access the full detail page for that specific test at /lab-results/:id.

Medications​

Active Medications​

Each active medication shows the drug name, dosage, how often to take it, who prescribed it, start date, which hospital it came from, and any related condition it is treating.

Click Ask ChartChat on any row to get a plain-language explanation of that medication.

Discontinued Medications​

Shown below active medications when applicable. Shows the drug name, dosage, date, and the reason it was stopped (if recorded).

Medication Detail​

The detail page shows all recorded information for that prescription, plus a Renew your prescription link that lets you submit a renewal request directly from the portal.

Conditions​

Active Conditions​

Displayed as cards. Each card shows the condition name, its medical code, when it was first diagnosed, whether it is chronic, and which hospital recorded it.

Click Ask ChartChat about this condition on any card to get a plain-language explanation.

Resolved Conditions​

Shown as a table below active conditions. Includes the condition name, diagnosis code, the period it was active, and the source hospital.

How to Filter​

  • Active / Resolved / All tabs
  • Search by condition name

Allergies​

When you have documented allergies, a warning banner appears at the top reminding you to inform your healthcare providers.

Layout​

Allergies are shown in two groups side by side:

Each allergy card shows the allergen name, how severe it is, the reaction it causes, and which hospital recorded it.

Procedures​

Completed Procedures​

Each row shows the procedure name, when it was performed, who performed it, the location, and a status badge. Click Ask ChartChat on any row for a plain-language explanation.

Ordered / Pending​

If you have any procedures that have been ordered but not yet performed, they appear in a separate section below. Urgent orders are highlighted in red.

Visits​

The Visits page has two sections: your upcoming appointments and your past visit history.

Summary Cards​

Upcoming Appointments​

Shows all your scheduled appointments with the date, time, provider name, clinic, and status.

Click any row to open the appointment details drawer which shows:

  • Full appointment information including any patient instructions
  • Add to Google Calendar button (for future appointments)
  • Ask ChartChat button to ask questions about the appointment

Past Visits​

Shows all your previous clinical encounters with the date, visit type, and provider name.

Click any row to open a quick-view drawer with a summary of that visit. From there you can:

  • Click View Full Visit to see all details on a dedicated page
  • Click Ask ChartChat to ask questions about that visit

Immunizations​

Medical Records​

This page brings together all your medical documents — clinical notes, lab reports, radiology reports, and discharge summaries — in one place.

Category Tabs​

Opening a Document​

Click any row to open the document in a viewer. ChartChat supports:
