Member Portal — Chat Assistant
Overview
The ChartChat AI Assistant is an intelligent chat interface that helps patients understand their own medical records in plain, everyday language. It is not a general-purpose medical chatbot — it works exclusively with the data already present in the patient's chart and does not provide medical advice, diagnoses, or treatment recommendations.
Disclaimer: ChartChat explains your existing medical data. It does not provide medical advice, diagnoses, or treatment recommendations. Always consult your healthcare provider.

How the AI Responds
Normal Response
The assistant replies with an explanation in plain language based on the patient's chart data.
Source Citations
When the assistant references specific records from the chart, a Sources from your chart section appears below the response, listing:
- Resource type (e.g.
Observation,MedicationRequest) - Record name and date
This allows patients to cross-reference the AI's explanation with the actual record in the portal.
Error Response
If the API is unreachable or returns an error:
"Sorry, I couldn't process your request."
Chat Sessions
- Sessions are stored and persist across logins
- Sessions are grouped in the sidebar by time: Today, Yesterday, This Week, Older
- On first load, the last 7 days of sessions are shown; older sessions load automatically as you scroll
- The session title is set to the first words of your first message
- You can rename or delete sessions from the sidebar
- Starting a New Chat creates a fresh session (no session is created until you send your first message)
Context-Aware Chat
When opened from a specific health record, the assistant is pre-loaded with context about that record:
| Source | Context passed |
|---|---|
| Lab result row | The specific Observation (value, reference range, status, date) |
| Condition card | The specific Condition resource |
| Visit row / drawer | The specific Encounter resource |
| Report / note row | The specific DiagnosticReport or DocumentReference |
| Medication row | The specific MedicationRequest resource |
