Hospital Integrations

What Is This Page For?​

Every hospital that patients can connect to in the member portal must be registered here first. When a patient wants to link their health records, ChartChat checks this list to know which hospitals are available, how to connect to them, and how to pull data on the patient's behalf.

If a hospital is not on this list, patients cannot connect to it.


Viewing Your Hospitals​

In the left sidebar, click Integrations → Hospital Integrations (or go to /hospitals).

You will see a table of all registered hospitals with these columns:

Searching and Filtering​

Use the filter bar at the top to narrow the list:

  • Search box — type a hospital name and press Enter
  • EHR Vendor — filter to show only Epic, Healow, or MEDITECH hospitals
  • Status — filter to show only Active or Inactive hospitals

All three filters can be used together. Clear any filter by clicking the × on its value.

Hospitals are shown 20 per page, sorted by most recently updated first.


Adding a New Hospital​

  1. Click + Add Hospital (top right of the list)
  2. Fill in the form — see Form Fields below
  3. Click Test SMART Discovery to verify the connection works before saving
  4. Click Create Hospital

The hospital will appear in the list immediately. If Enabled is checked, patients can connect to it right away.


Editing a Hospital​

  1. Click the pencil icon on the hospital's row
  2. Make your changes
  3. Click Save Changes

Changes take effect immediately. If you update the connection URL or credentials, existing patient syncs will use the new values on their next scheduled run.


Enabling and Disabling a Hospital​

The toggle switch in the Status column lets you turn a hospital on or off without deleting it.

Use this when:

  • A hospital's credentials need to be updated and you want to take it offline temporarily
  • You are setting up a new hospital and want to test it before making it live

Deleting a Hospital​

Click the red trash icon on the hospital's row. A confirmation box will appear with the hospital name and the message "This cannot be undone." Click Delete to confirm, or Cancel to go back.

Hospital deletion is permanent. If patients have already connected to this hospital, their health records will stop syncing. Always prefer disabling (toggling off) over deleting. Only delete if you are certain this hospital connection will never be needed again.


Form Fields​

Hospital Name (required)​

The name patients will see when browsing hospitals to connect in the member portal. Use the hospital's official name as patients would recognise it — for example Northwell Health or Cleveland Clinic.

EHR Vendor (required)​

The software system this hospital uses. ChartChat needs to know this to connect correctly.

FHIR Base URL (required)​

This is the technical web address ChartChat uses to connect to the hospital's system and pull patient records.

Where to get it: Ask the hospital's IT or integration team for their "FHIR R4 base URL". You do not need to construct this yourself — the hospital provides it.

After entering the URL, always click Test SMART Discovery before saving (see below).

Test SMART Discovery​

This button checks whether the connection URL actually works. Click it after entering the FHIR Base URL.

  • Green result — the URL is working. You are safe to save.
  • Red result — the URL is not working. Do not save until this is resolved. Contact the hospital's IT team to confirm the correct URL.

If the connection test fails and you save anyway, patients will get an error when trying to connect to this hospital.

Practice Code (Healow hospitals only — required)​

A short code that identifies your practice within the Healow system. eClinicalWorks provides this code when they set up your integration.

You can usually find it in the FHIR Base URL itself — for example in https://healow.com/apps/fhir/**JAFJCD**/api/FHIR/R4, the practice code is JAFJCD.

Client ID (MEDITECH hospitals only — required)​

A unique identifier the hospital gives ChartChat when registering it in their system. Each MEDITECH hospital has a different Client ID. The hospital's IT team provides this.

Client Secret (MEDITECH hospitals only — optional)​

Only needed for certain MEDITECH hospitals that require extra security on the connection. If you are unsure whether this is needed, ask the hospital's IT team. Most do not require it.

OAuth Scope (optional)​

Controls exactly what health data ChartChat requests permission to access on the patient's behalf.

Leave this blank — ChartChat automatically uses the right settings for each vendor. Only fill this in if the hospital specifically instructs you to use a custom scope list.

Redirect URI (optional)​

The web address the hospital's system sends the patient back to after they log in and approve access.

Leave this blank — ChartChat has built-in defaults that work for most hospitals. Only fill this in if the hospital's IT team tells you that a specific redirect address is required.

Logo URL (optional)​

A web link to the hospital's logo image, shown to patients in the member portal when they browse hospitals. The image must be publicly accessible on the internet.

Recommended: a square or landscape image on a white or transparent background.

Enabled​

Controls whether patients can see and connect to this hospital.

  • Checked (default) — Hospital goes live immediately after saving
  • Unchecked — Hospital is saved but hidden from patients. Use this when you are not ready to make it live yet.

Step-by-Step: Adding an Epic Hospital​

  1. Click + Add Hospital
  2. Hospital Name — enter the full hospital name, e.g. Mount Sinai Health System
  3. EHR Vendor — select Epic
  4. FHIR Base URL — enter the URL provided by the hospital's IT team
  5. Click Test SMART Discovery — wait for the green confirmation
  6. OAuth Scope, Redirect URI, Logo URL — leave blank unless you have specific values
  7. Enabled — leave checked to make it live immediately
  8. Click Create Hospital

Step-by-Step: Adding a Healow Hospital​

  1. Click + Add Hospital
  2. Hospital Name — enter the practice or clinic name
  3. EHR Vendor — select Healow (eClinicalWorks)
  4. FHIR Base URL — enter the URL provided by eClinicalWorks
  5. Practice Code — enter the practice code from eClinicalWorks (e.g. JAFJCD)
  6. Click Test SMART Discovery — wait for the green confirmation
  7. Click Create Hospital

Step-by-Step: Adding a MEDITECH Hospital​

  1. Click + Add Hospital
  2. Hospital Name — enter the hospital name
  3. EHR Vendor — select MEDITECH
  4. FHIR Base URL — enter the URL provided by the hospital's IT team
  5. Client ID — enter the client ID the hospital issued for ChartChat
  6. Client Secret — enter only if the hospital's IT team says it is required
  7. Click Test SMART Discovery — wait for the green confirmation
  8. Click Create Hospital

Monitoring Hospital Sync Health​

Once hospitals are set up and patients start connecting, you can monitor whether their records are syncing correctly from Reports → EHR Integration Health (/reports/ehr-health). This shows which patients are syncing successfully and which have issues, grouped by hospital.


Troubleshooting​

Test SMART Discovery shows a red error​

The connection URL is not working. Before saving:

Do not save the hospital until the test passes.


A patient cannot connect to a hospital​

  1. Check the hospital's toggle is green (active) in the list
  2. Click the edit icon and re-run Test SMART Discovery — confirm it passes
  3. If the test fails, update the FHIR Base URL with the correct address from the hospital's IT team
  4. Check the Audit Log (/activity-log) for any error events for that patient

Patients' records stopped updating​

  1. Go to Reports → EHR Integration Health and check the sync status for affected patients
  2. If the patient's token has expired, they need to reconnect in the member portal under Settings → Connected Providers — admins cannot do this on their behalf
  3. If all patients from one hospital are failing, the hospital may have changed their system credentials or URL. Open the hospital, update the details, and re-run Test SMART Discovery

A hospital was accidentally deleted​

Hospital deletion cannot be undone. You will need to re-add the hospital using the + Add Hospital form with the same details. Patients who were connected to it will need to reconnect after the hospital is re-added.
